Clubs & Centers should use this form to indicate team and scrambler chaperone & members. Clubs & Centers should fill out this form as many times as necessary, once for each team, scrambler, or group of scramblers.

The chaperone is the name, cell & email requested as the registrant. The chaperone will receive an automated email from the system upon this registration and a reminder email prior to the rally. 
Signed Chaperone Forms can be scanned and emailed to the Rally Secretary or snail mailed to the Rally Secretary, Jen Watson at 35121 Scotland Heights Rd, Round Hill, VA  20141 or jenjohnwatson@yahoo.com

Payment can be made electronically via invoice from individual registrations or snail mailed with a check made payable to VRPC to the VRPC Treasurer, Vicki Golden 10811 Warwick Ave., Fairfax, VA 22030.
